HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?
If you are on regular medication you may obtain repeat prescriptions with the doctor’s agreement. WE DO NOT ACCEPT REQUESTS BY TELEPHONE. TO AVOID RUNNING SHORT OF YOUR MEDICATION, PLEASE ENSURE YOU MAKE YOUR REQUEST IN PLENTY OF TIME. You need to allow two clear working days (excluding Bank holidays and weekends) if collecting the prescription from our surgery and at least five clear working days if using the services of a local pharmacy, to allow for collection, dispensing and delivery in good time.
ON-LINE REQUESTS:
You can request your prescriptions on-line but first you will need to attend
the surgery with photo ID eg passport, driving licence, freedom pass where you
will be given a personal password and log-in.
PHARMACIST LED PRESCRIPTION SERVICE:
You can present your repeat prescription request (the tear off slip that comes with your prescription) to your local chosen pharmacy. They will request the repeat on your behalf, collect the prescription from our surgery and dispense your medication. You can arrange with them to either collect the medication yourself from their premises or have it delivered to your home. Their services are free of charge. REMEMBER TO ALLOW AT LEAST FIVE WORKING DAYS FOR THIS SERVICE.
FAXED REQUESTS:
You can fax your prescription request to the surgery on 020 7771 4025.
REQUESTS BY POST:
Post your repeat prescription slip to the surgery enclosing a stamped, addressed envelope or place it in the repeat prescription box at reception. Many local chemists operate a collection scheme. Please ask your chemist or the reception staff about this. From time to time your doctor may request you to make an appointment to have your medication reviewed - please check for messages/due dates on your repeat prescription slip.
HOW DO I....
OBTAIN A SICKNESS CERTIFICATE?
For employees, a sickness certificate completed and signed by the doctor is only required for a period of continuous absence from work due to ill health for eight days or longer. For an absence of up to three working days no certificate is required. Between three and seven days absence due to illness is covered by a self-certification form (SC1 or SC2) provided by your employer. These statutory DSS rules are universal and all employers are aware of them. If you require a sick note for insurance purposes etc, the doctor will be able to issue a private certificate for which there is a charge, currently £15.00.
We do not issue private certificates for the benefit of employers for periods
of absence less than a week.
